State police issued a traffic ticket Tuesday to the truck driver who is accused of causing the accident on June 12 that killed a Carthage teenager and injured another.

Joshua R. Girard, 34, County Route 75, Adams, is cited with a stop sign infraction, and is ordered to appear in town of Denmark Court.

Police said the investigation is continuing in consultation with the district attorney's office, leaving open the possibility of additional charges being filed.

Police said Mr. Girard was northbound on East Road in a Lowe's Home Improvement Warehouse truck when at about 4:20 p.m. he apparently failed to see a stop sign.

The rig entered Route 410, colliding with a westbound minivan operated by Scott L. Benson, Carthage. Mr. Benson's daughter, Mykayla L. Benson, 13, was killed.

Another passenger, Kendra L. Kehoe, 13, Black River, was taken to Upstate Medical University, Syracuse, where on Tuesday she was listed in fair condition.
