The family of Agnes Ann McKithen, 45, of Timmonsville, say they have questions about what exactly lead to her death on Thanksgiving Day at QVC Distribution Center on TV Road in Florence.

Florence County Coroner Keith von Lutcken said McKithen was 30 feet in the air on a forklift when it was hit by another forklift inside the plant.

He said McKithen's forklift overturned, causing her to fall. Lutcken said she died at the hospital.

Her family was home preparing Thanksgiving dinner when they got the call about the accident.

McKithen's husband Charles Carter, Jr. said they had only been married eight months.

"Never expected this. Like I said when she left that morning, it was our first Thanksgiving. You know and I'm sitting here looking at the clock and 2:45 never came. Not for us to start our first Thanksgiving. Never came. What did come was tragedy, heart break, disappointment," said Carter.

McKithen's loved ones said she was a joy to be around and always did what she could to help others.

"Agnes, if you knew her was a die hard QVC employee. She was a workaholic. She did what she had to do. And for an accident like that to happen to her, it's unbelievable. And yes, we want answers. We want to know exactly what happened to cause our loved, our sister, our daughter, our grandmother, our cousin, our friend to die. This shouldn't have happened. Doing the thing she loved to do most. At the prime of her life," said Brenda McKithen.

"To lose a sister Thursday, Thanksgiving Day, it took part of me with her. I always vowed to protect her. And I felt like I failed because I wasn't there to catch her when she fell," said James McKithen, Jr.

The funeral service for McKithen will be Tuesday at 2 p.m. at the New Zion Baptist Church and burial will be in Mt. Carmel Baptist Church Cemetery, according to Bacote-Eaddy Funeral Home of Timmonsville.

Lesia Kudelka with the SC Department of Labor, Licensing and Regulation said SC Occupational Safety and Health Administration (OSHA) are investigating the accident that killed McKithen. She said the investigation could take up to eight weeks.

QVC released the following statement to WPDE on Thursday, Dec. 4: "We express deep sorrow and sadness over the devastating loss of Agnus McKithen, following a forklift accident at our Florence Distribution Center. Our thoughts and prayers are with her family of all those affected by this tragic death."
