Rachel Renee (Kelley) Slavens, 26 of Sapulpa, OK, formerly of Columbus, KS died Sunday, Dec. 8, 2013 at St. John's Hospital in Sapulpa following a sudden illness.
Funeral services will be 2 pm, Saturday, December 14, 2013 at the Bath-Naylor Funeral Home in Columbus with Pastor Jim Louis officiating. Burial will follow in the Lone Elm Cemetery.
