Chad Falck, age 44, of Alta, Iowa died Sunday, August 21, 2016 in rural Storm Lake, Iowa.

Funeral services will be held Friday, August 26, 2016 at 10:30 a.m. at St. John Lutheran Church in Storm Lake. Burial will follow in the Buena Vista Memorial Park Cemetery in Storm Lake. Visitation will take place Thursday, August 25, 2016 from 3-8:00 p.m. with the family present from 6-8:00 p.m. at the Fratzke & Jensen Funeral Home in Storm Lake. In lieu of flowers, a memorial fund has been established for his son, Holton's education. The Fratzke & Jensen Funeral Home in Storm Lake is in charge of the arrangements.
