A 23-year-man died and three children and a woman were hurt in a two-car crash Monday evening, police say.

Luke Kuryluk, of Parma Heights, was pronounced dead at the scene of the crash that happened shortly before 7 p.m. on Snow Road near State Road, according to a news release from Parma police.

The 36-year-old woman in another car car suffered serious injuries and three of the six children in her car were hurt, the release says.

Paramedics took the woman and her children to MetroHealth for treatment. All of the children in the car ranged from 1 to 11 years old. Their current conditions are not known.

A passerby who was also a nurse revived a 1-year-old who stopped breathing at the scene, police said.

Kuryluk drove his 2008 Subaru Outback west on Snow Road when he crashed head-on into a 2012 Honda Pilot, according to police reports.

Kuryluk veered into the eastbound lanes and struck the Pilot, police said. What caused him to cross the center lane remains under investigation.

Firefighters cut the door and windshield of Kuryluk's Subaru to get him out, the release says.

The crash is under investigation.
